FERTILTY ENHANCING ENDOSCOPIC SURGERY

Treatment of fertility doesn’t always involve medicines or assisted reproductive techniques, endoscopy has a major role to play in diagnosis and treatment of many fertilty related problems. Hysteroscopy and laparoscopy are used many a ways to enhance fertilty results ,known as fertilty enhancing endoscopic surgery.

A. Laparoscopy for infertility

It helps to detect many pathologies like

  • Blocked fallopian tubes
  • Hydro salpinx
  • Endometriosis
  • Fibroid, Ovarian cyst
  • Other anatomical abnormalities of the reproductive systems.

It can also been used as  therapeutic measures for many infertility conditions.

1.hydrosalpinx

Salpingectomy before ivf increases 2 fold chance of pregnancy .

2.Ovarian drilling

Used in severe PCOS cases, helps to decrease  the androgen level and increase the chance of conception after ovulation induction.

3.endometriosis

In grade 3 or grade 4 endometriosis, debunking of pathology and adhesionolysis followed by IVF is the key of success to parenthood.

4.myomectomy

Increases the chances to restore the uterine cavity and increases the pregnancy rate.

5.Using laparoscopy In case of any suspicious growth, a biopsy can be taken during laparoscopy.

Procedure

Here 1 camera is inserted through the umbilical making a 10 mm entry in the abdomen and as per need 1 or 2 side port are created to enter the instruments and to carry out the procedures making 5mm cut on abdomen. The recovery is very quick, performed under general anaesthesia as a day  care procedure.

B. Hysteroscopy

Its a procedure in which a device is inserted through the vagina into the patient’s uterus to check for abnormalities.

  • Indications:
  • Evaluation of patients trying to conceive assess uterine volume.
  • Exploration of the endocervical canal and uterine cavity in patients with repetitive pregnancy loss.
  • Diagnosis and possible removal of growth inside the uterus like fibroid, polyp.
  • To remove any adhesions which were caused due to an infection or a surgery.
  • To take a biopsy of your uterine lining to see the evidence of genital tuberculosis.

PROCEDURE

The hysteroscope is inserted into the patient’s vagina to view the uterus properly. The instrument is moved inside to get clear images of the uterus and the opening of the fallopian tubes.The bilateral ostia should be at same level of the uterine funds.

Hysteroscopy for diagnosing problems takes very less time when compared to hysteroscopy for surgeries and operations. It is  a day care procedure which is done under analgesia. Patient may experience some spotting after the procedure but recovery is excellent .
